Clinical Pharmacy is an integral part of the Pharmaceutical Department activity and is a challenge for the pharmaceutical profession. The pharmacist began to focus his activity on the patient and not just on the medicine. Thus, it is responsible for identifying, preventing and resolving Drug Related Problems (PRMs). In order to improve patient care, it applies its technical and scientific knowledge to the assessment and identification of pharmacotherapeutic problems, suggesting changes to it. Identifies and addresses deficiencies in patient knowledge and education regarding their disease and pharmacological therapy. It enables the pharmacist to actively participate in improving adherence to therapy and contributing to beneficial health outcomes

Clinical Pharmacy. Concepts. Basic Functions in Clinical Pharmacy: Selection of medicines. Committee on Pharmacy and Therapeutics. National Medicine Formulary. Drug distribution. Medication information. Interpretation of laboratory data and clinical significance. Pharmacist´s intervention (topic and clinical cases): Oncology. Breast cancer. Preparation of cytotoxics. Hematology. Neonatology. Chronic kidney disease. Specialized Nutritional Support - Parenteral and enteral nutrition. Preparation of PN in a real work context. Cardiac insufficiency. Dyslipidemia. Rational use of antibiotics. Pharmacovigilance. Special populations: geriatrics, pediatrics and pregnant women. Clinical pharmacokinetics. Clinical Trials. Elaboration and presentation of group work related to the topics under study.

Objectives
Objectives
Acquisition of a critical and analytical attitude to the detection and resolution of pharmacotherapeutic problems affecting each patient. Acquisition of necessary tools for the practice of pharmaceutical care: how to collect and interpret the patient's clinical history, interpretation of laboratory parameters and knowledge of the pharmacotherapy clinical guidelines. Pharmacist as part of the multidisciplinary healthcare team. Understanding and applying the principles of pharmacokinetics in individualizing dosage regimens. Awareness of the importance of collaborating in programs that have the purpose of detecting, reporting, recovery and prevention of adverse drug reactions. Integration of concepts from studies of drug use, to apply to the selection of drugs, based on criteria of effectiveness, safety, and cost-effectiveness.
